| Sports physicals and health insurance are required in order to try-out and participate in sports at Neil Armstrong. Sports physicals are valid for two years from the examination date. Sports physicals are available at the School-Based Health Center. For appointments, please call 503-359-4057. If your child needs health insurance, check out the Healthy Kids program at www.OregonHealthyKids.gov or call 503-359-0446 to speak to a local representative. | |

Registration and sports physical forms may be downloaded using the above links or picked up from the main office.
Boys' Basketball will practice from 2:45 to 4:30, Monday through Friday. Track and Field will begin on Monday, March 18. Please make sure you have a current sports physical and the registration packet completed prior to this date. Registration packets can be picked up from the main office or downloaded from this website. |
